Create is a strong word in Hebrew. It's the verb used in Genesis 1 for God's original work — bringing something out of nothing. David is asking for new construction inside.
A clean heart isn't a slightly improved heart. It's a different heart. Only God can do that kind of work, and David knows it.
Then he asks for a renewed spirit — the inner posture, the right attitude. New heart on the inside, right spirit in the daily life.
Remember when he prayed this. After his worst moment — adultery, cover-up, murder. If David can pray for a clean heart from there, you can pray it from wherever you are today.
